If you are a current or former registrant of the Alberta College of Dental Hygienists (ACDH or CRDHA or ADHA), you can request a Letter of Good Standing to be sent to another regulatory body or other institution. To initiate your request, please complete the online request form (see below).
In the majority of instances, the regulatory body or other institution requires you to submit a specific form. Please have it completed, including your signature, and upload it within the online form.
Important Information
- Processing Time: Requests are typically processed within 15 business days. All requests are handled in the order they are received.
- Fee: A fee of $25 applies for each Letter of Good Standing, payable by credit card during the online application process. Processing of your request begins as soon as we receive payment.
Submission Details
- Letters are sent via email to the recipient(s) and copied to the requester. A hard copy is sent to the recipient(s) shortly after.
- If you need to change the recipient or required documentation after processing, you must submit a new request.
Before You Submit
Make sure you have all required information ready. The ACDH is not responsible for confirming the details that you have provided in your request.
If you are requesting for another regulatory body, confirm the following:
- Does the body require the letter to be sent to them from the ACDH?
- If yes, enter the regulatory body as the recipient and provide their contact details in the form
- If no, choose either:
- "Applicant" as the recipient if you want the letter sent to you
- The regulatory body as the recipient if you want the letter sent directly to them.
- Does the body also require a separate form to be completed by the ACDH?
- If yes, download a copy of the form from the body, complete the first part (including your signature), and have the full form ready to upload in your request.
If you are requesting for another institution, confirm the following:
- Does the institution require the letter to be sent to them from the ACDH?
- If yes, enter the institution as the recipient and provide their contact details in the form
- If no, choose either:
- "Applicant" as the recipient if you want the letter sent to you
- The institution as the recipient if you want the letter sent directly to them.
- Does the institution also require a separate form to be completed by the ACDH?
- If yes, download a copy of the form from the institution, complete the first part (including your signature), and have the full form ready to upload in your request.
